Aides at Home participates with the following programs and provides Personal Care Aide, Home Health Aide, Home Care Services, and Nursing services to make possible for you or your loved ones to continue living in the community. Aides at Home supports the needs of patients with positive, empowering approach that celebrates independence, dignity, and quality of life.
You must be approved by one of these programs to receive services from Aides at Home.
MLTC
Managed Long Term Care
Patients that have Medicare and Medicaid (dually eligible individuals) and require long-term care services and support at home.-

MCO
Managed Care Organization
Medicaid managed care plan delivers Medicaid health benefits and additional services through contracted agencies.-

CHHA
Certified Home Health Agency
Program provides short-term and intermittent skilled services to patients at home.-

WAIVER
TBI – Traumatic Brain Injury
NHTD -Nursing Home Transition and Diversion
Medicaid waivers are for people with disabilities that require long term care at the comfort of their home

Hospice
Home Hospice Care
Hospice care focuses on terminally ill patients and attends to their personal, emotional and spiritual needs in comfort of their home.-

DSS
Department of Social Services
Program provides an alternative to entering an institution and promotes independence in a home setting.-

EISEP
Expanded In-home Services for the Elderly
The program provides non-medical services such as housekeeping, personal care, respite, case management and related services.-

Private Duty
Privately paid program provides certified caregivers.
